Austin Health is the major provider of tertiary health services, health professional education and research across 3 campuses in the northeast of Melbourne and state-wide. Austin Health is world-renowned for its research and specialist work in cancer, liver transplantation, spinal cord injuries, neurology, endocrinology, mental health and rehabilitation.
Have you ever wanted to know more about the care needs of chronic and complex clients either before they present to the emergency department, or after they return home from hospital? This short term position offers an exciting opportunity for an enthusiastic and experienced RN Clinical Nurse Consultant, Grade 4 to join the Health Independence Program (HIP), specifically within the Chronic Heart Failure Care Management services area.
This role is vital in contributing to our overall HIP objectives; providing support so people can continue to live at home, and reducing unnecessary re-presentations and re-admissions to our health service.

This 24 hours/3 day per week position, is a part time, fixed term, leave cover position from 30th March - 14th May 2021, and reports to the Team Leader - Complex Care Management (HIP).
