Position Summary

Austin Health is world-renowned for its research and specialist work in cancer, liver transplantation, spinal cord injuries, neurology, endocrinology, mental health and rehabilitation.

 

An exciting fixed term full-time or part-time opportunity currently exists for an enthusiastic and highly experienced Registered Nurse or Allied Health Professional to join our Health Independence Program. Term: October 2021 - 30 June 2022.

 

The Health independence Program provides a range of services that deliver time-limited healthcare to support the client's transition from hospital to home. These services can also prevent the need for a hospital presentation or ward admission, with people able to access these services directly from within the community.

 

Details of position: The Complex Care Manager (RN Grade 4A or Allied Health Grade 2) ensures a timely response to the complex needs of the HIP eligible individual who is at imminent risk of presenting to the Emergency Department or representing after a recent hospital discharge. The care manager provides service in a variety of settings including the Emergency Department, the client's home, and within the community or ambulatory setting.

 

To be successful in this role you must have:

  • At least five years relevant and recent clinical experience
  • Well-developed comprehensive complex care assessment, care plan development and evaluation skills
  • Proven ability to problem-solve in a variety of complex situations
  • Ability to communicate effectively in both wiritten and verbal form
  • Ability to work effectively as part of a dynamic team, within a multi-disciplinary environment
