Position Summary

Austin Health is the major provider of tertiary health services, health professional education and research across 3 campuses in the northeast of Melbourne and state-wide. Austin Health is world-renowned for its research and specialist work in cancer, liver transplantation, spinal cord injuries, neurology, endocrinology, mental health and rehabilitation.

An exciting opportunity currently exists for an enthusiastic and highly experienced Registered Nurse Grade 3B or Allied Health Professional Grade 2 to join the Health Independence Program (HIP) Central Intake & Fast Track Care Management team.

Fixed term Part Time 30.4 hours per week (7.6 hours x 4 days); 11th November 2019 to 26th January 2020.

Thereafter a reduction in hours, Fixed term Part Time 15.2 hours per week (7.6 hours x 2 days); 27th January 2020 to 24th January 2021.

This position contributes to the overall HIP objective - to support people to remain living at home and to prevent unnecessary presentations to the Emergency Department or admission to Austin Health. The Central Intake and Fast Track Care Manager (RN Grade 3B or Allied Health Professional Grade 2) works with clients post hospital admission, or referred from the community, providing a fast track (rapid) response to identify client needs, develop and implement care plans and monitor clients through to discharge from the HIP service. The position is primarily based at the Heidelberg Repatriation campus of Austin Health however will require travel across campuses.

Selection Criteria Essential: • Registered Nurse Division 1 (with current AHPRA registration) or Allied Health Professional (with current AHPRA registration as applicable to discipline) with eligibility for membership of relevant professional association. • At least 5 years relevant clinical experience. • Client-centred approach
