Position Summary

Austin Health is the major provider of tertiary health services, health professional education and research across 3 campuses in the northeast of Melbourne and state-wide. Austin Health is world-renowned for its research and specialist work in cancer, liver transplantation, spinal cord injuries, neurology, endocrinology, mental health and rehabilitation.

An exciting opportunity exists for an enthusiastic and highly experienced Project Officer in our Strategy, Planning and Integrated Programs department to plan, implement and evaluate a coordinated care project for frail older people and people with complex and chronic needs. This position will see the project officer:

  • Lead the co-design of a new model of care, in close partnership with relevant stakeholders. 
  • Develop a tool to facilitate proactive identification of Austin Hospital inpatients with frailty and/or complex conditions.
  • Implement the new model of care.
  • Evaluate the effectiveness of the proactive identification system and the model of care.

To succeed in this role, the project officer will have:

  • Relevant tertiary qualification or experience in a health or related discipline.
  • Demonstrated success in implementing change management projects in a clinical environment.
  • Experience or knowledge of project management methodologies.
  • Understanding of acute health services and of the acute/community interface.
  • Experience developing partnerships between multiple stakeholders.
  • Ability to build trust and credibility, and to develop and maintain constructive relationships.
  • A ‘can-do’ attitude.
  • Highly organised, sets realistic deadlines and manages a busy work plan.
  • Reflects and seeks feedback on performance and impact.
