• Adult Community Mental Health - Case Manager
• Based in Heidelberg
• Monday to Friday - 8.30am-5:00pm
• Permanent full-time or part time positions available! 
• Open to: Registered Psychiatric Nurse (Div1) Qualified Social Worker, Occupational Therapist, Clinical Psychologist
• Member of the North East Area Mental Health Service (NEAMHS) Continuing Care Service

CCS is a multidisciplinary team of highly passionate, experienced, supportive, collaborative and enthusiastic mental health professionals. Our values underpin everything we do. We are seeking applicants for a full-time or part-time permanent Case Manager. This role is a unique opportunity to have a real impact in meeting the needs of one of our community’s most complex and vulnerable cohorts. You will form solid working-relationships within the pre-existing community teams, and foster strong therapeutic relationships with your clients. We support rehabilitation goals through the use of best evidence-based frameworks and assertive case management.

Your contribution:

• You are a team player with a commitment to quality client care
• You will have a professional qualification specific to your discipline 
• Clinical skills to perform comprehensive Mental State Examinations, Risk Assessments, Therapeutic Interventions
• Minimum 3 years’ experience in a relevant mental health setting
• Demonstrated knowledge of recovery and collaborative clinical practice.
• A working knowledge of the Mental Health Act (2014), the Carer Recognition Act (2012), and Chief Psychiatrists Guidelines
