Position Summary

Austin Health is the major provider of tertiary health services, health professional education and research across 3 campuses in the northeast of Melbourne and state-wide. Austin Health is world-renowned for its research and specialist work in cancer, liver transplantation, spinal cord injuries, neurology, endocrinology, mental health and rehabilitation. 

An exciting opportunity currently exists for an enthusiastic and highly experienced Diabetes Nurse Educator to join our Diabetes Education team. 

To be successful in this role you must demonstrate/Key responsibilities will include:

Provide high quality diabetes education for both inpatients and outpatients

Work well in a multidisciplinary team environment

Work in conjunction with the Diabetes Education Service Administration Clerks to ensure that all paperwork, bookings and filing are completed in an efficient and timely manner

Assume responsibility for own continuing education requirements 

Participate in professional activities to enhance knowledge and experience to improve health care delivery and patient outcomes

Provide follow up phone support for patients in collaboration with the wider diabetes team

Recommend and participate in the updating of policies and procedures relevant to the management of diabetes

Attend and contribute to endocrine ward rounds

Understand and confidently use latest technology for the management of diabetes including insulin pumps and continuous glucose monitoring and flash glucose monitoring systems

Provide high quality and age-appropriate diabetes education services for paediatric and young adults with diabetes and their families/support persons with a demonstrated understanding of the complexities of these age groups

Provide support for discharge planning and referral to outpatients, Diabetes Care-Management Assessment Service (DCAS) or community services as applicable
