An exciting opportunity for a Clinical Nurse Consultant (Registered Nurse) to join the Residential In-Reach Team in a full or part time fixed term position.

About the In-Reach Team

The Residential In-Reach Team (RIR) is located at the Austin Hospital Campus of Austin Health and is tiered within the Continuing Care Clinical Service Unit. The service is funded via the Department of Health (DoH), Victoria, for the purpose of reviewing people living in residential aged care facilities (RACFs), to provide a timely, person-centred management approach for acute medical conditions.

Service Aims:

  • To support residential aged care facility staff to manage acute care of unwell residents
  • To provide an alternative to hospital-based management of acute conditions
  • To reduce avoidable transfers to the Emergency Department (ED) from residential aged care


  • Registered Nurse (Division 1) with current AHPRA registration
  • Relevant post graduate experience in Acute Care Nursing/ Aged Care Nursing
  • Advanced assessment skills
  • Excellent team working skills, with ability to work independently
  • Demonstrated safe problem-solving skills, with the ability to use the principles of nursing practice
  • Enthusiasm, willingness to learn
  • Developed computer literacy skills and familiarity with Office 365
