An exciting opportunity currently exists for an enthusiastic and highly experienced Mental Health Clinician to join our North East Area Mental Health Service - Continuing Care Service. 

Details of position/position requirements: 

Community Mental Health Case Manager at the North East Area Mental Health Service
Hours of operation are Monday - Friday 8:30am - 5:00pm.

Mandatory Qualifications - relevant professional qualification in a health related discipline (Psychology, Psychiatric Nursing, Social Work, Occupational Therapy)

A minimum 3 years of experience in a relevant mental health setting.
Demonstrated knowledge of recovery and collaborative clinical practice.
A thorough working knowledge of the Mental Health Act (2014), the Carer Recognition Ace (2012), and Chief Psychiatrists Guidelines

To be successful in this role you must demonstrate/Key responsibilities will include:

  • Displays a clear understanding of the principle of recovery orientated practice and its implementation within a clinical setting.
  • Ensure the delivery of a high standard of evidenced-based care to consumers
  • Comprehensive Mental State Examinations, Risk Assessments, Physical Health Monitoring, Psychosocial Assessment and Rehabilitation Goals, Psychotropic Medication Knowledge 
  • Collaborate with interagency and external agencies to ensure the consumer’s safe and effective transition across the care continuum (including Emergency Services, Department of Health and Human Services, Corrections and Specialists)
  • Identifying and responding to the deteriorating patient 
  • Formulation of treatment plans 
  • Ensure continuity of care to consumers, families and carers through the principles of case management.
  • Participate in service development by providing liaison, consultation and education services for other health care professionals, consumer and community groups and agencies.
  • Provide knowledge and skills, based on professional background, as part of a multidisciplinary team, consult with other North East Area Mental Health Service (NEAMHS) staff on specialist and community psychiatry matters.
  • Provide timely provision of all service activity data (Rapid and Outcome Measurement), including contact details and other statistical data as requested by the NECCS Manager and Mental Health Division management.
  • Engage in professional development activities as directed.
  • Assist with NE CCS Duty function as required.
  • Undertake in other duties that may be required as may arise in the course of employment period.
